　バセドー病（Basedow's disease）などの甲状腺機能亢進症における眼症状のひとつに上眼瞼後退による瞼裂の開大がある。この眼瞼後退はDalrymple's signならびに，Graefe's signの原因と考えられている。グアネチジン（guanethi-dine）のような交感神経遮断剤の点眼投与によって病勢の軽快することが知られているが，現在我国ではこの薬剤は入手困難である。そこで我々は今回本症を呈した患者12症例に，β遮断剤（チモロール，ベフノロール，カルテオロール）の点眼を行ったところ，この上眼瞼後退が3例において軽減した。

　その奏効機序として，本症患者における眼瞼後退は，上下の瞼板筋（ミューラー筋（Müller'smuscle））の過剰な緊張によるものと考えられ（Rundleら），β遮断剤はその交感神経遮断作用により，交感神経性に支配される瞼板筋の痙攣が軽減するため，瞼裂開大が軽快したものと考えられた。

We treated 12 patients with thyroid ophthal-mopathy with beta-blocker ophthalmic solutions instilled twice daily. All the patients manifested widening of the palpebral fissure due to upper lid retraction as manifestation of the disease. We em-ployed timolol befunolol or carteolol in the treat-ment. Three patients responded favorably to the treatment. Significant improvement in upper lid retraction set in after 3 weeks to 3 months of treatment in them.

Upper lid retraction in hyperthyroidism is claimed to result from hypertoxicity of superior and inferior tarsal muscles which are under sympa-thetic innervation. The sympatholytic action of beta-blockers would reduce the spasm of the tarsal muscles and induce improvement in the widened palpebral fissure.
