Topical instillation of epinephrine induced prompt and sustained control of intraocular pres-sure (IOP) in 3 cases with primary open angle glaucoma after the eyes poorly responded to topical beta-blockers. This finding seemed to illustrate that the response differs for each invididual : some are highly sensitive to epinephrine and some to beta -blockers. As a noteworthy feature in the present series, the hypotensive effect with epinephrine last-ed as long as 10 years, in sharp contrast to gradual reduction in the effect as is often seen with beta -blockers.

While we chiefly used topical 1-epinephrine, we also observed promising effects of topical dipive-frine which is a prodrug of epinephrine.
