眼球突出を伴わない一側の瞼裂開大症について

Unilateral lid retraction without exophthalmus

緒言

　眼球突出を伴わない1側の瞼裂開大症に就ては本邦に於ては，河本，島崎両氏の報告があり，河本氏（1926）は文献的に考察して本症には2種あり，1は先天性に属し生来存せるもの，2は後年に何等の理由なくして来るものにして，その原因は全く不明となしバセドー氏病とは区別しておられる。島崎氏（1932）は本症例に関する報告は僅少であつて内外を通じて僅か10例余りにすぎないようであると述べ，同氏の自家経験例では全植物神経の軽度の緊張亢進は認めるが，基礎代謝率が＋10〜＋12％である処から甲状腺機能亢進なしとしてバセドー氏病を否定している。ところがMcLean及びNorton等（1959）は本症例12例に就て甲状腺機能異常を精査したところ，5例は明かにtoxicであり，4例はeuthyroidではあつたが，1年後にはtoxicとなつた。残りの3例は，toxicityの証拠はなく，このうち2例はtriiodo-thyronine suppression testの結果は陰性又は（±）であつた。而して同氏等は本症は他に説明がつけられないのでPochinと同様，本症例の症状は甲状腺系の機能異常の或る型の顕症であると主張している。Werner （1955）は例えeuthyroidpatientでも早期眼症状を有するものは，triiodo-thyronine suppresion testをすれば病的反応を来すという。

The functions of the thyroid system were examined in 2 cases which showed the clinical picture of unilateral retraction of the upper lid of one eye without other ocular abnormali-ties. Both cases had no struma. The first case was a 28 years old female. Basal metabolic rate and I¹³¹ uptake rate were normal, and autonomic amphotony was diagnosed by auto-nomic nerve function tests. The second case was a 41 years old female. Basal metabolic rate and I¹³¹ uptake rate were accelerated and sympathicotony was diagnosed by the same tests.
