Abstract 文献概要

　1982年1月から1984年12月までに名市大未熟児病棟で管理された出生体重1,500g以下の極小未熟児96例の院内出生児と院外出生児を対象として，死亡率と網膜症の発生・進行について比較検討し，次の結果を得た．

（1）院内出生児は33例，院外出生児は63例で死亡率の差はない．

（2）網膜症は院外出生児に有意に多く発生し，重症例も院内出生児よりも院外出生児に多い．

（3）出生体重750〜1,249gの院外出生児とその3期以上の発生は院内出生児よりも多い．

（4）網膜症の発生と進行は出生体重の増大に伴い院内出生児では急激に減少し，院外出生児では徐々に減少する傾向がある．

（5）死亡率と網膜症の発生，進行について出生体重，在胎期間，唐木らの未熟指数Pを比較すると出生体重と最も高い相関があり，P値とは院外出生児でわずかに相関するが，院内出生児では相関がない．これはP値が院外出生児のみから算出された値であるためと推測される．

（6）院内，院外共に新生児仮死や入院時チアノーゼの強いものは死亡しやすく，院外出生児で入院時に低体温のあったものは3期以上の網膜症を生じやすい．

We evaluated a consecutive series of 96 infants over a 3-year period (1982-84). They weighed 1,500 g at birth and were admitted to the neonatal care unit of our hospital.

The present series consisted of two heterogenous groups. There were 33 infants who were born in our hospital (Group A) and another 63 cases who were referred to us after birth elsewhere.

There was no difference in the survival rate between the two groups. The incidence and severity of retinopathy of prematurity (ROP) were significantly higher in Group B than in A. The incidence of stage 3 and more severe ROP was strikingly high in infants with birthweight of 750 to 1,249 g belonging to Group B. The incidence and severity of ROP decreased, alongwith the increase in birthweight, rapidly in Group A and gradually in Group B.

Throughout the series, the birthweight was more significantly correlated with the rate of mortality and aggravation of ROP than the gestational period or the prematurity score P after Karaki.

We evaluated the possible role of degree of Apgar score, cyanosis and hypothermia on the occurrence and progression of ROP. Cases with hypothermia in Group B showed a high incidence of severe ROP.

Above findings indicate that transportation of pre-mature infants to neonatal care units involves inherent risks regarding the eye. We emphasize that maternal transportation is to be preferred to neonatal transporta-tion when risk factors are involved.
