極小未熟児の増加と網膜症の発生，進行に関する統計的研究

Statistical analyses of incidence and progression of retinopathy of prematurity in relation to increased occurrence of very low birthweight infants

　名市大未熟児病棟で管理された生下時体重2,500g以下の低出生体重児のうち，すでに報告した1970年1月から1975年6月までの470例（A群）と，1977年1月から1979年12月までの195例（B群）を対象として，未熟児網膜症の発生，進行について比較した。さらに，B群における1,250g以下の極小未熟児について，網膜症と全身的諸因子との関連性を検討し，以下の結果を得た。

（1）名市大では極小未熟児が増加しており，特に1,250g以下が著明である（p＜0.001）。

（2）それに伴って重症網膜症も増加している。

（3）Ⅱ型網膜症は約5倍に増えたが，統計学的には有意な増加とはいえない。

（4）全身管理法は，名市大においてはA群の時期にすでに現在の水準まで達していたと考えられ，発生率と重症例の増加は，極小未熟児の増加によるために他ならない。

（5）生下時体重1,250g以下において，（a）Ⅰ型網膜症は全身的諸因子に関係なく発生，進行している。（b）Ⅱ型および混合型は，全身的諸因子に強い関連性があり，ほとんど全ての条件が悪い症例に発生している。

（6）網膜の未熟性こそ本症の重要な発生因子であることを再確認した。

（7）本症による失明予防には，極小未熟児の出生防止が急務である。

Two groups of low birthweight infants were evaluated as to retinopathy of prematurity based on records of our NICU. Group A consisted of 470 infants admitted during the 5 years 6 months ending June 1975 and Group B of 195 infants during the 3 years 1977-79. There was a significant increase in the incidence of very low birthweight infants in Group B. The increase was particularly pronounced in the incidence of those with birth-weigt of 1,250g or less (p<0.001) and those with the gestational period of 30 weeks or less (p< 0.001).
