斜視外来における改良型Fundus Haploscopeの有用性について

Advantages of the fundus haploscope in clinical ophthalmology

　Fundus Haploscopeは大型弱視鏡の機能を備え，内蔵された赤外線カメラにより眼底を同時に観察できる器械であり，その特徴は眼底と視標との位置関係が明確にできる点にある。しかしこの器械は大型であり，日常の臨床には使用しにくい欠点がいくつかあったので，臨床での使用を目的としてFundus Haploscopeの改良を行った。1）上下斜視に対処するため，鏡筒の上下への傾き，すなわちあおりができるようにした。2）スライドの上下左右への移動や回旋を，手動から電動にきりかえた。3） TVの画像を水平方向40°垂直方向30°と広くし，眼底の位置関係をわかりやすくした。4）ピント調節により眼底や視標の大きさが変化しないようにした。

　このような改良により，以前よりも簡単に操作できるようになったので本器械を大型弱視鏡と同様に斜視外来で使用し，その有用性を1）検査可能年齢，2）網膜対応異常症例について検討した。その結果，3,4歳では検査可能な症例は．大型弱視鏡とくらべると少ないが，5歳以上では90％以上の症例に対し使用可能であった。また網膜対応の異常な症例は全体の17％にみられ，そのうち融像まで得られた症例は，すべて微小角斜視であることが確認された。

　Fundus Haploscopeは改良により操作しやすくなり，研究面のみでなく，日々の診療に十分使用可能となった。

The Fundus Haploscope fulfills the functions that can be performed by any conventional major amblyoscope available so far. Fundus pictures taken by the infrared TV cameras included in it are shown on the TV monitor simultaneously with the target slides. By using the Fundus Haploscope, the relationship between the fundus and the target in respect to their location can be precisely defined.

But there were still some points to be improved upon. Consisting of 2 conventional fundus cameras, the whole unit was rather oversized and expensive. Operation was rather complicated and the optic tubes could not be elevated or depressed.
