光凝固の奏効した虹彩転移癌

A case of metastatic carcinoma of the iris successfully treated by argon laser photokoagulation

　右眼虹彩および隅角への転移癌の1例を，報告した。当初，腫瘍を含む虹彩全幅切除術を施行したところ2日後に再発をみた。その後．新たに出現した多数の転移巣に対し，アルゴンレーザー光凝固術を繰り返し施行し，一時的にではあるが，腫瘍を細隙検査上消失せしめた。病理組織では左肺原発巣に一致する小細胞未分化癌の浸潤を，虹彩に認めた。虹彩転移癌に対しては，虹彩切除術は手術操作により，腫瘍細胞を前房内に撒布する危険がある。初期段階にアルゴンレーザー光凝固を施行すると，腫瘍の消退に非常に効果的であり，視力の保持も可能であった。虹彩隅角部転移癌に際してはまず光凝固を施行し，必要であれば手術操作を施行すべきと考える。

A 69-year-old female developed visual impair-ment and injection in the right eye. She was under treatment for small-cell carcinoma in the left lung for the past 8 months with cytostastics and radia-tion. Three small whitish gray nodules were located on the iris at the 10-11 o'clock position in the right eye. Additionally, three smaller nodules were pres-ent in the chamber angle in the same sector. The corrected visual acuity of the affected eye was 0.3. The left eye was intact.

We performed total iridectomy and sclerectomy including the tumors. The resected tissue showed the same cell type as in the lung.
