網膜芽細胞腫の放射線療法について

Treatment of retinoblastoma by radiation therapy

　過去5年間に治療した網膜芽細胞腫23例27眼の臨床所見と経過について，網膜芽細胞腫の治療における放射線療法の臨床的意義につき再検討した。放射線療法をしなかった症例に眼窩内再発をみとめたり，眼球摘出に放射線療法を併用した群でも3例に死亡例をみとめた。病理組織学的には23例中4例に眼窩内浸潤をみとめた。両眼性の症例では，第1眼を摘出し第2眼に放射線療法を行った症例では，白内障がみられたが，眼球摘出せずにすんだ。腫瘍塊が小さければ，放射線療法と光凝固僚法による保存的療法が有効であった。

Retrospective studies were made of the thera-peutic effects of radiation therapy in 27 eyes of 23 cases of retinoblastoma which we treated during the past five years.

Patients were grouped into 4 groups :

Group 1 : enucleation only (monocular retino-blastoma) (2 cases)

Group 2: enucleation and postoperative ir-radiation with ⁶⁰Co (monocular reti-noblastoma) (16 cases)

Group 3: enucleation of the first eye and ir-radiation of the second eye (bilateral retinoblastoma) (3 cases)

Group 4: irradiation combined with photoco-agulation (one monocular and one bilateral retinoblastoma) (2 cases)
