Paracentral serous chorioretinopathyを併発したfundus albipunctatusの1症例

Occurrence of paraccntral serous chorioretinopathy in a fundus albipunctatus

　Fundus albipunctatusに傍中心性網脈絡膜症を併発した症例を報告した。症例は34歳の男性で，飛蚊症のため精査を希望し来院された。眼底には，大きさ一定の黄白色小斑点が，黄斑部を含む後極部から赤道部まで散在していた。諸検査の結果，典型的なFundus albipunctatusの症例と診断された。

　定期的に経過観察を行っていたところ，初診から1年3カ月後に，耳側赤道部に限局性の漿液性網膜剥離をきたし，螢光眼底造影にて，その部に1個の点状螢光漏出点が認められた。アルゴンレーザー光凝固を行い，症状は軽快した。

　Fundus albipunctatusの病理組織学的な検索はこれまでになされていないので，この疾患の本態については憶測の域を出ない。螢光眼底所見から網膜色素上皮の障害は論じられていたが，本症のようにneuro-sensory retinaの限局性剥離を併発する例があることは，本症の本来の病変である斑点の存在，あるいは色素上皮の変化の拡大により，色素上皮のbarrier機能に破綻を生ぜしめたことが推定できる。

　Fundus albipunctatusにこのような2次変化を来たした症例報告は，これまでにないが，本症例も病巣が黄斑部から離れているために自覚症状を欠き，定期的な眼底検査で偶然発見されたものであった。詳細な眼底検査により，同様な変化の検出は，もっと高率にみられるものと期待される。

A 42-year-old male was diagnosed as fundus al-bipunctatus with classical features of the disease. Fluorescein angiography showed fairly diffuse, ir-regular and spotty hyperfluorescence over the whole fundus, indicating impaired retinal pigment epithelium. The white spots blocked the hyperflu-orescence in the background.

During a follow-up examination 15 months later, a localized, flat detachment of the neurosensory retina was detected temporal to the macula in the right eye. Fluorescein angiography revealed aspotty dye leakage from the choroid in the center of the detached area.
