外傷性三角症候群

Traumatic triangular syndrome

　外傷性三角症候群の症例6例につき，眼底所見，螢光眼底所見を経時的に観察し，下の以知見を得た。

　（1）6症例全例が，後極部，乳頭に接した部に網脈絡膜萎縮をきたし，この病巣部は，外傷性三角症候群の特徴と考えられる。

　（2）6症例中2例に脈絡膜血管由来の網膜下新生血管の発現をみた。うち1例は，新生血管がfoveaに向い拡大し重篤な視力障害を残した。

　（3）6症例中3例に，網脈絡膜萎縮巣に隣接する部の網膜血管透過性亢進が認められた。このような症例の病巣部では，網膜静脈の閉塞，網膜毛細血管板の破壊が認められた。

　（4）受傷後，長期にわたる経過観察により，二，三の所見の早期発見と，適切な処置を行うことが大切である。

we investigated 6 cases of traumatic triangular syndrome with fluorescein angiography. All of them had juxtapapillary lesions of triangular shaped chorioretinal atrophy. This triangular lesion adja-cent to the disc was one of the characteristic patterns of vascular disturbance of the choroid following ocular contusion.

Subretinal neovascularization was observed in two cases by fluorescein angiography. In one of them, neovascular lesion gradually developed towards the macula and visual acuity was dimini-shed from 1.0 to 0.1. This finding may suggest the relative hypoxia in the macular even in juxta-papillary choroidal lesion.
