急性期血管新生緑内障（neovascular glaucoma）の治療

Retinal cryotherapy for the treatment of acute stage of neovascular glaucoma

　隅角閉塞の軽度なneovascular glaucomaを2症例経験した。第1例は，糖尿病のコントロール不良で，片眼は糖尿病性網膜症の増殖性変化が強くほぼ失明状態であり，健眼にneovascular gla-ucomaを発症した。日々に症状は増悪し，内服薬，点眼薬ではコントロールができない状態となり，止むを得ず網膜周辺部冷凍凝固術を行ったところ著明な眼圧の下降をみた。更に後日汎網膜光凝固術を追加して1.2の視力を保っている。第2例は，一眼は視神経萎縮で手動弁，他眼は原因不明の硝子体出血で6ヵ月前より手動弁であった。vitrectomyを行なって視力がでてきたが術後13日目にneovascular glaucomaを発症した。内服薬，点眼薬で眼圧の降下を計っていたがrubeosisは増強するので，第1例と同様に周辺部網膜冷凍凝固術を行なった。術後経過は順調で0.5の視力を維持している。

　まだ隅角閉塞の完成されていない上記のような時期では，適当な治療により眼圧上昇をコントロールすることができる可能性があり，neovas-cular glaucomaの急性期と考えた。眼圧上昇はrubeosisによる房水産生量の増大とplasmoidaqueousが主因と考えた。

We treated two cases of neovascular glaucoma with acute onset. The first case, a diabetic with poor blood sugar control, came to us with high intraocular pressure, (43 mmHg) complaining acute deterioration of visual acuity. There was severe neovascularization in the iris and chamber angle in his left eye with turbid aqueous probably due to high protein content. In spite of full medication, the intraocular pressure was uncontrollable and the vision deteriorated from 0.4 to 0.1. His right eye was blind. To rescue this cornered situation weperformed cryotherapy not to the ciliary body but to the retina.
