Neovascular glaucomaに対する治療の検討

Surgical treatment for neovascular glaucoma

　糖尿病性網膜症10例11眼，網膜中心静脈閉塞症5例5眼，他疾患群（Coats病，硝子体手術後等）11例11眼に続発したneovascular glaucomaに対し種々の治療を施行した。

　治療法としては，視力保存を目的とする場合には，早期にpanretinal photocoagulation，panretinalcryocoagulationにより，網膜のhypoxiaを改善し，trabeculectomyを中心とする観血的手術を実施すべきである事，視力回復の可能性のない例には，cyclocryotherapyが適当と考えられる。

　原因となった基礎病変により予後が異なり，網膜中心静脈閉塞症の予後が最も良く，次いで，糖尿病性網膜症で他の疾患群の予後が最も不良であった。

We evaluated the efficacy of various therapeutic modalities in a consecutive series of 27 eyes with neovascular glaucoma. As the underlying lesion, diabetic retinopathy was present in 11 eyes, central retinal vein occlusion in 5 and other lesions (Coats' disease, post vitrectomy etc) in 11.

The most favorable results were obtained with trabeculectomy combined with panretinal photo-coagulation or cryoapplication. Cyclocyotherapy was also of value in bringing the glaucoma under control in eyes with poor outlook for recovery of vi-sion.
