強膜ブドウ腫をきたしたvon Recklinghausen病の1例

Posterior scleral staphyloma as a complication von Recklinghausen disease

緒　言

　Von Recklinghausen病（以下R病と略す）は，全身の末梢神経に多発性の神経線維腫を生ずる系統的な疾患で，眼科領域においても，1863年Billroth以来多数の報告があり，その眼症状も極めて多彩で^1,2），眼瞼，虹彩，毛様体，脈絡膜，網膜，視神経，眼窩などの各部位に神経線維腫が形成されてくることは良く知られている。しかし，R病が水眼症を合併することは比較的稀とされており，本邦では，1908年河本^3）が記載して以来，数例の報告があるが，強膜ブドウ腫をきたした症例は，文献的にもこれをみない。

　われわれは，R病によつて強膜ブドウ腫をきたした1例を経験し，その発生機転について考察を加えたので報告する。

A 65-year-old male with von Recklinghausen's disease had manifested café-au-lait spots and hy-podermic nodules on the body surfasce since child-hood. During the past 30 years, he developed gradually progressive exophthalmos of the right eye associated with visual impairment and later in blindness.

Clinical and anatomical studies of the excised eye showed the clinical symptoms to be due to staphyloma developing in the posterior portion of the eyeball. The posterior scelra appeared very thin and was infiltrated with tumor cells.
