Congenital Angiodysplastic Exudative Vitreoretinopathy

緒　言

　網膜血管の未熟性を基盤にして，これに酸素が一つの引き金となつて未熟児に発生する未熟児網膜症は，一つのclinical entityとして確立されている。一方，網膜症が，酸素を使用しない例，たとえば成熟児に稀に起こることも指摘されている。Brockhurstら^1）は瘢痕症例の観察より，酸素非使用の成熟児に未熟児網膜症と全く同様の眼底所見を呈する症例を報告している。しかしこの成熟児にみられる網膜症の機序，病態は未熟児網膜症と全く同じものなのか，あるいは臨床所見が類似しているのみで本態は異なるものなのかについては末だに明らかにされていない。今回著者らは著明なvitreo-retinopathyを呈する症例を経験し，特にそのうちの2症例について初期の所見より瘢痕に至るまでの全経過を，螢光眼底検査を含めて詳細に観察した結果，これらの疾患は酸素非使用であること，満期産，成熟児であるにもかかわらず網膜血管の著しい未発達が認められたこと，網膜は汚臓色調を呈し低形成ないしは変性を思わせる所見を呈していたこと，滲出性増殖性変化の進行とともに硝子体網膜牽引，網膜剥離，黄斑偏位などの所見を呈したこと，家族性発症のみられないことなどより未熟児網膜症，familiar exudativevitreo-retinopathy，Coats'disease，PHPVなどとは異なつた新しい独立した疾患であると結論するに至つた。

We observed three infants each of whom ma-nifested peculiar vitreoretinopathy in one eye and congenital leucocoria in the fellow eye.Either of the three cases were born full term and did not receive oxygen after birth. While the observed virtreoretinopathy showed certain simi-larity to retrolental fibroplasia, familial exuda-tive vitreoretinopathy and Coats' disease, the natural history and the overall picture of the cases seemed to be well distinct from these hitherto documented diseases.

During the early stage, a wide avacular area involved the entire peripheral fundus.
