黄斑部孔を伴う網膜剥離に対する黄斑プロンベの試作とその手術法について

Use of a Special Macular Explant in Surgery for Retinal Detachment with Macular Hole

はじめに

　黄斑部孔を伴つた網膜剥離の治療法は，光凝固療法のよい適応とされているが，それでも安静位をとらせて剥離を消失させ，光凝固が可能となる症例は少なく，あらかじめ何らかの観血的な手術方法にて，先ず剥離網膜を復位させねばならぬ場合が多い。この際にも，網膜下液を単に穿刺排液するのみでは復位は得られにくく，剥離網膜の復位を強制する何らかの補助手段を構じねばならぬ場合が多い。

　われわれは今回，手術方法が簡単で，黄斑部をある程度永続的にbuckleできるexplantとして，黄斑プロンベ（仮称）を作り，数例に使用してよい結果を得たので報告する。

Five cases of retinal detachment arising from macular holes have been operated on by buckl-ing procedure using a macular plombe (Ando).

This plombe is made of silicone plate which includes a stainless steel wire to maintain a cer-tain form to indent macular area. The plombe is 21 mm long, 5 mm wide and 2 mm thick, and has a button of 5 mm in diameter and 1.5 mm in height at its top in order to indent ma-cular area.

The surgical procedure is easier than the direct fixation of an explant or Klöti's silver clip and more reliable than the scleral infolding me-thod.
