網膜色素変性症に対する抗凝血剤Indionの臨床的効果について

The effect of phenylindandione on the visual field in pigmentary degeneration of the retina (a preliminary report)

　Phenylindandioneは又一名をPhenindioneフエニンジオン（N．N．R．）とよばれる薬剤であつて，化学構造的には2-phenyl-v-1，3-indandioneで示される無臭の淡黄色結晶である。さて本薬剤の臨床的効果については多くの研究がなされている。即ちKabat，Stohlman，Smith氏等は1944年にIndandione （命名は1947年）は抗凝血剤として臨床的に使用出来ることを報告した。そして同氏等はIndandioneがProthrombin産生を阻止する作用を有することを報告し，且その効果がDicumarolより急激であることを述べた。又Chalmer，Dixon氏等により1954年に本薬剤はVitamin Kに拮抗する作用を有することが認められている。本薬剤が臨床的に登場したのは抗凝血剤として，血管内に於ける血液凝固が病因となる疾患，心梗塞，脳栓塞，肺栓塞等に対しての効果が期待されたからであつた。さてこのような血栓形成阻止剤としては本剤に加えてHeparin，クマリン誘導体が挙げられることは周知である。そして本剤の主作用は血漿中の凝血因子であるProthrombinと安定因子の生成を減少せしめ，血液の凝血能を低下させるものである。

The author applied anticoagulant phenylindandione on 10 cases of pigmentary degenera-tions of the retina.

Visual fields of 6 cases out of 10 were dramatically enlarged after 2 weeks of dral oraly administration of 50mg phenylindandione. The effect of this drug is still unknown, but lipolytic factor may be effective for the degeneration of the retina.

Vasodilatasive and anticoagulant property of this drug may also be efficient for the small nutrient vessels of the retina.
