A New Biomicrogoniophotography. Report 1. Critical Appraisal of Hitherto Current Methods

緒言

　眼科診療において隅角鏡検査が重要であることは論をまたない。特に緑内障の診断，マネージメントには不可欠な検査となつている。この隅角鏡検査は隅角に関する大まかな構築上の情報をもたらすものである。

　たとえば隅角が広い，狭い，色素沈着がある，先天性遺残がある等の所見にとどまるもので，実際に房水の主流が通る隅角壁およびそれより末梢の流路に関する情報は得ることができない。広隅角緑内障における流出抵抗の場は隅角壁より末梢にあるはずであるから，現在の隅角検査法ではなんらの情報も得ることができないことになる。近年の光顕，電顕，走査電顕の進歩はその部分の構造をあきらかにしつつある。しかしいずれも"biomicro"所見ではなく，流路の病態に関しては全く不明である。

An analysis of optical features of Koeppe-and Goldmann-type was carried out as a preli-minary step to the construction of an improved setup for biomicrogoniophotography.

1. Spot-diagram analysis of Koeppe- and Gold-mann-type goniolenses, when combined with the Gullstrand model eye, has shown that certain aberrations do exist. The estimated aberrations were of similar order for both types and were independent of the angle of the incident ray.

2. 4.8% of the incident ray was lost before reaching the chamber angle and was converted into flare or ghost of the gonioscopic image.
