放射線障害による絶対緑内障眼に見られた前房隅角部変化の電顕所見

Fine Structure of Trabecular Meshwork of Absolute Glaucoma Following Irradiation Therapy of Malignant Tumor

緒言

　眼部または上顎部悪性腫瘍の治療に放射線照射がしばしば行なわれるが，後に不快な副作用の一つである緑内障を生ずることがある^1）。その発生機序はいまだ不明であるが，房水排出路の構造になんらかの変化の生じていることが予想される。すでに光顕レベルで，放射線照射後の緑内障眼には虹彩隅角部癒着や虹彩前面のfibrovascularmembraneの形成が見られたという報告^2）があるが，trabecular meshworkおよびそれ以後の扉水流通路の構造変化，ことに微細構造についてはほとんど知られていない。この報告では，Co⁶⁰照射後絶対緑内障に陥つた2症例より得た眼球の虹彩隅角癒着部およびtrabecular meshworkの電顕的研究を行なうことにより，その機序解明の手がかりとしたい。

A light-and electron-microscopical study was made on two human eyes with absolute glauco-ma which followed the irradiation therapy of mali gnant tumor in the orbital or maxillary region.

Neovascularization of the iris and synechial angle closure were chief microscopical features. Electronmicroscopically, the uveal and corneoscle-ral trabecular meshwork presented various stru-tural changes. The intertrabecular space between the cords or sheets had disappeared in some areas.
