緒言
著者は先に網膜剥離を治療法との関連に於て分類することを提唱した。それは「24〜48時間絶対安静後の"残存網膜下液"」の量により，網膜剥離を5段階に分け，その各階級毎に適応手術法を選択すべきであるとするものである1）。すなわち第1度の網膜剥離は残存網膜下液が全くないもので光凝固の適応である。第2度の剥離とは残存網膜下液が硝子体容積の2〜3％まででジアテルミー穿刺凝固の適応となる。第3度剥離は残存網膜下液が10％程度までで強膜短縮術の適応であり，第4度剥離は10％以上でSchepensのシリコン埋没並びに緊縛法が適応となる。第5度は綱膜下液が更に多量（硝子体容積の1／4或いはそれ以上）で，スケペンス法と共に硝子体内人工房水注入が必要なものである。ここに重症網膜剥離というのは第4度及び第5度の剥離を指して言い，こうした症例に有効なSchepensのシリコン埋没並びに緊縛法と，硝子体内人工房水注入法の術式を簡単に説明し，同時に若干の症例を紹介するのが，この報告の目的である。
An improved surgical approach is necessary for cases of retinal detachment with highly vaulted retina and scanty tendency of reattach-ment, as usual methods of detachment surge-ry, as simple retinopexy through perforating diathermy and even scleral shortening opera-tions, prove often to be inadequate or inefficient.
The present authors are performing, for these instances, the scleral buckling procedures of Schepens with the use of silicone strips and tantalum clip. In the presence of massive vit-reous retraction, the techniqueis often supple-mented by the intravitreal injection of artificial aqueous humor.
