網膜剥離に対する流体シリコン注入後の経過

A Long-Term Follow up of Silicone Retinopiesis for Retinal Detachment

緒言

　重症の網膜剥離に対する手術のひとつとして，流体シリコンを硝子体内に注入する方法SiliconeRetinopiesisは，Stone （1958），Armaly （1962），Cibis （1962）等以来諸家により推進されたが，術効は一時的で永続性のないことと合併症の点からもまたその評価は必ずしも高くはない。しかし元来，もはや希望がないとみられるほどの難治性のものに対する手段であるからには，たとえごく少数例ではあつても長期にわたり予後佳良なものがあるとすれば，それなりの価値は認めてやらなければなるまい。われわれ^1）が20例の経験を第21回臨床眼科学会に発表したのは1967年秋のことであるが，その後，5年余を経過したので，ここに続報として長期追跡の結果を報告する。

Retinal detachment surgery was performed in 34 eyes using fluid silicone as vitreous sub-stitute. The surgical outcome after a longer follow-up period was favorable in 3 eyes, fair in 10 and ineffective in 15. All the cases hap-ped to be advanced or complicated ones for which other routine surgical procedures were not indicated.

Chief late complications consisted of cataract (5 eyes), glaucoma (5 eyes), iridocyclitis and/ or vitreous opacity (6 eyes) and migration of silicone into the anterior chamber (4 eyes).Removal of silicone from the anterior chamber through aspiration resulted in improved clinical course in 2 instances.
