裂孔位置による網膜剥離の四分分類法—（その1）周辺群網膜剥離の分析

Four Group Classification of Rhegmatogenous Retinal Detachment according to the Location of Retinal Breaks

緒言

　「裂孔原性網膜剥離は網膜視部（pars optica）に裂孔を生じ，その結果これが色素上皮から剥離してくる疾患である」という定義の上では裂孔原性網膜剥離は単一疾患であるように見える。しかしこれは表面上のことで，さらに裂孔の成因にまで遡つて考える時，その原因は決して単一でなく種々異なつた機序が働いていることは明らかである。従つて裂孔原性網膜剥離は種々の疾患の複合であると考えた方がよい。それ故にこそ裂孔原性網膜剥離はこれまでにも種々に分類されたし^1），著者自身も1971年に主として裂孔の位置と形を基準にした原因別分類を試みた^2）。しかしこれまでの分類では著者自身のものも含めて分類基準に原理的な統一が欠け，ある場合には外傷や水晶体摘出などという偶発的外因を分類基準としてとるかと思えば，同時に格子状変性や硝子体剥離などという本質的かつ内因性の変化をも分類基準にするという具合であつた。そのため同一症例が各群に重複して所属したり，反対に所属する分類がない症例が現われたりする有様だつた。

The cause of retinal breaks in rhegmato-genous retinal detachment was thought to be different according to the location of breaks and, therefore, the author proposed to classify the rhegmatogeneous retinal detachment into four groups as to macular, intermediate, peri-pheral and oral. He examined 600 consecutive eyes with retinal detachment that were operated by himself and found that the periphery group, in which retinal breaks were situated between the line of posterior border of vitreous base andthe line of 17 to 19mm behind the limbus, was the largest groups including 440 eyes, 73% of all.
