網膜剥離術後の再発と家族性滲出性硝子体網膜症による網膜剥離

Recurrence after surgery for retinal detachment in eyes with familial exudative vitreoretinopathy

　過去32か月間に当科で手術を行った裂孔原性網膜剥離のうち，40歳以下の症例145例156眼を対象として，家族性滲出性硝子体網膜症による網膜剥離と網膜格子状変性円孔による網膜剥離について，発症年齢，初回術式と再発率を比較検討した。両群の割合は，前者が26例28眼（18％），後者が64例73眼（47％）であり，平均発症年齢は前者で16.7歳，後者で25.0歳であった。初回手術後の再発率は前者が28眼中5眼（18％），後者が73眼中3眼（4％）と前者が有意に高かった。前者の再発5症例は，全例が周辺網膜無血管野の新裂孔より再発し，5回再発を繰り返した1例では術後の光凝固が再発の原因となった。したがって家族性滲出性硝子体網膜症に併発した網膜剥離で再発を防止するためには，全無血管野を強膜バックリングと輪状締結術による隆起の上にのせることと，網膜過凝固を避けることが必要と考えられた。

We reviewed a consecutive series of 503 eyes in 488 patients operated by us for rhegmatogenous retinal detachment during a 32-month period end-ing August 1989. There were 145 patients under the age of 40 years in the series, of which 64 patients, 73 eyes, were associated with lattice hole and 26 patients, 28 eyes, with familial exudative vitreo-retinopathy. The age of onset was younger in thelatter. Postoperative recurrence was more frequent in the latter at 5 eyes, 18%, than in the former at 3 eyes, 4% (p＜0.01). The recurrence in all the 5 eyes with familial exudative vitreoretinopathy was due to newly developed retinal breaks in the peripheral avascular retina. We advocate wide episcleral buckling and encircling procedure which would cover all the avascular areas in the peripheral retina.
