網膜剥離における硝子体内生食注入の手術成績

Intravitreous salin injection in retinal detachment surgery

　単純な裂孔原性網膜剥離に対してsegmental bucklingを行った症例に排液後生食もしくはBSS （Balanced Salt Solution）を硝子体内に注入した症例の手術成績を検討し，以下の結果を得た。

（1）網膜裂孔の閉鎖が確実であれば，硝子体内に注入された生食もしくはBSSが網膜下へ入り込むことはなかった。

（2）術後の網膜の機能回復に対する影響はないものと考えられ，合併症もほとんどなかった。

　以上の結果より，網膜剥離の手術において生食などの硝子体内注入は，網膜裂孔の閉鎖が確実であれば有効な手技であると考えられた。

We surgically treated 11 cases with uncom-plicated rhegmatogenous retinal detachment with segmental scleral buckling with release of subretinal fluid and intravitreous injection of saline. Trans-scleral cryoapplication was performed in all the cases. All the operated eyes resulted in reattach-ment of the retina with favorable recovery of retinal function during the average follow-up period of 9 months.

The injected saline did not flow into the sub-retinal space through the retinal break whenever the break was properly closed. The intravitreous saline did not apparently interfere with the reco-very of the retinal function.
