網膜剥離に対する流体シリコンの応用

The Application of Liquid Silicone for Retinal Detachment

I．はじめに

　網膜剥離の手術の際に，流体シリコンを硝子体内に注入しようとする試みは，Stone （1958），Armaly （1962），Cibisら（1962），Levin＆Ellis （1963），Niesel＆Fankhauser （1964），わが国においては，伊藤・久保田（1964），岸本・森（1966），田原（1967）らによつて推進せられ，最近ではWatzke （1967）が長期観察の成績を述べている。

　報告者の1人，小島（現姓，森本）（1966）はさきに，家兎の前房および硝子体に流体シリコンを注入し，臨床的ならびに組織学的所見に基づき，この種流体形成資材の眼内諸組織に対する障害性を検討し発表した。その成績によれば，硝子体注入の場合，実験動物のおよそ1／3の例数において1〜2カ月後に，水晶体後嚢附近にわずかながら混濁を生じたが，その後のいちじるしい進行はなかつた。硝子体混濁も一部に生じたが，軽度であり，後には吸収された。網膜の組織学的検索では，1年を経過せる例にあつても，周辺部にて内外顆粒層に配列の乱れを認めた程度に過ぎなかつた。そして，以上により，流体シリコンの組織障害性は皆無とはいえないまでも，実地臨床の応用には可能であろうと推論した。

Twenty cases of complicated retinal detach-ment were treated by injection of liquid silicone into the vitreous cavity with or without evacu-ation of subretinal fluid. Of the 20 patients, 4 had secondary detachment associated with vit-reous contraction following the perforative ocular injury, and 16 had idiopathic detachment. The latter included 2 patients of aphakic eye, 6patients who showed no improvement for pre-vious operations by the usual means, and 13 patients with no definite retinal tear.
