開眼検診における角膜移植の適応の選定について

The screening examination of the blinde. A false suggestion of the keratoplasty

　光のプレゼント運動がマスコミで取りあげられこれに共鳴して貧困者の開眼手術の費用を拠出せんとする篤志的な動きが芽ばえている。従来から行われていた開眼検診に実を結ばせ，文字通り盲者に光明を与えるものであるから，誠に有意義な運動であるが，これに関連して最近遭遇した出来事を記し，かかる運動をさらに盛りたてるうえで開眼検診についての私見を述べたいと思う。

　昭和36年7月，本学某教授の紹介で，関東周辺にある○市の有力者の来訪を受けた。過日○市で開眼検診が行われた結果，角膜移植の適応と診定された7名は医療保護の対象となる人達なので，市の予算で医療費をまかない開眼手術を施したいとの要件で，その協力依頼であつた。そこで，非常に結構な主旨ではあるが御了解願わねばならぬ事項として，「角膜移植は100％の成功を保証し難い」，「合併症の有無によつて成功率も異るしまた手術回数が1回ですむとは限らない」，「移植角膜入手の関係から，限られた期間内での実施を保証し難い」ことを述べ，手術の正確な適応をきめるために施術前に一度，精密検査を行つたほうがよい旨を説明した。

In the medical examination of the blinde made in a local city, five patients were sug-gested the possibility to improve vision by the keratoplasty. They visited the author's clinic accompanied by a boss of the city in the full hope to restore vision.

Except one case of the corneal macula after interstitial keratitis 4 patients had too bad conditions to undergo the keratoplasty successfully : one case (both eyes) of anterior sta-phyloma, two cases of the heavy adherent leucoma accompaied with secondary glaucoma, and one case of the dense leucoma spread over the almost whole cornea.
