無水晶体眼における角膜乱視と矯正眼鏡レンズ—第1報Ophthalmometerで測定される角膜乱視と矯正円柱レンズ度の換算

Corneal Astigmatism and the Correcting Spectacle Lens in Aphakia.:Report I. Calculation of the Correcting Cylinder Lens Power from the Corneal Astigmatism Determined with an Ophthalmometer

緒言

　Ophthalmometerは今日，コンタクトレンズ装着の検査に主用されているが，歴史的にみると1881年Javal et Schiötz^1）による発表以後生まれた各種の器種は，角膜乱視の測定を眼の全乱視に結びつけ，乱視検査の簡易化に役立たせんとされた時代が続いた。現在でもophthalmometer各器種の測定目盛には曲率半径mmとともに角膜屈折力Dが刻まれ，なかにはD目盛のみを具えた器種もあるが，有水晶体眼においては水晶体乱視未知のまま，角膜乱視から全乱視を推定しても大近似に止まるところから，乱視検査へのophthal—mometerの活用は次第に等閑視されるに至つた。そして角膜乱視測定が的確に矯正眼鏡レンズの選定に役立つはずの無水晶体眼に対しても，ophthalmometerはさして活用されていないように思われる。

　無水晶体眼の乱視は，角膜乱視だけで構成されると考えることができるが，それを眼鏡矯正する場合，無水晶体眼が通例強度遠視であるだけに，レンズ角膜頂点問距離の存在を無視できないはずであるにもかかわらず，文献を渉猟した範囲ではその検討が十分に行なわれているとは思えず，この度その問題に関して臨床的に有用と思われる知見を得たのでここに報告する。

Optical problems necessary to utilize ophthal-mometry for the determination of the astigma-tism of the aphakic eye were studied. The re-fractive index used for the computation of the corneal refractive power graduated on the oph-thalmometric scale differs from 1.332 to 1.3376. Such difference in the index was ascertained to give negligible error to the estimation of the corneal astigmatism.
