結膜乳頭腫の擦過標本においてヒトパピローマウイルスを検出した1例―Hybrid capture Ⅱ assayの応用

A case of human papillomavirus with positive findings in the smear of conjunctival papilloma. Application of hybrid capture Ⅱ assay

要約　目的：結膜乳頭腫の擦過標本にヒトパピローマウイルス（HPV）が検出された症例の報告。症例：58歳男性が2か月前からの次第に増大する瞼結膜の隆起を主訴として受診した。所見：視力，眼圧，眼球については異常がなかった。右眼の下眼瞼に血管が豊富で有茎性の腫瘤があった。腫瘤を摘出し，冷凍凝固を行った。病理組織は結膜乳頭腫であった。Hybrid capture Ⅱ assayで低リスク型HPVに対するrelative light unit値が腫瘍塊と擦過標本でともに陽性であり，高リスク型HPVに対するそれは陰性であった。初診から1年後の現在まで再発はない。結論：本症例ではHPVが結膜乳頭腫の発症に関与している可能性が高く，hybrid capture Ⅱ assayはHVPの診断に有効であった。

Abstract.　Purpose：To report a case of conjunctival papilloma with positive findings for human papillomavirus（HPV）in the conjunctival smear by hybrid capture Ⅱ assay. Case：A 58-year-old male presented with slowly enlarging tumor in the right lower palpebral conjunctiva since 2 months before. Findings：Routine ophthalmologial findings were normal including corrected visual acuity,intraocular pressure,slit-lamp microscopy and funduscopy. The right eye showed a vascularized,pedunculated tumor in the lower palpebral conjunctiva. The tumor was resected followed by cryocoagulation. Histopathology showed the tumor to be conjunctival papilloma. The tumor mass and conjunctival smear showed positive values for low-risk human papillomavirus by hybrid capture Ⅱ assay. Values were negative for high-risk human papillomavirus. There has been no recurrence for 12 months until present. Conclusion：This case illustrates that human papillomavirus may be causatively related with conjunctival papilloma and that hybrid capture Ⅱ assay may be useful in the diagnosis.
