ローリスク群ヒトパピローマウイルスが検出された結膜乳頭腫の1例

Detection of human papillomavirus in a case of conjunctival papilloma

要約　背景：結膜の乳頭腫と扁平上皮癌との臨床的な鑑別は困難である。目的：結膜乳頭腫からヒトパピローマウイルスが検出された症例の報告。症例：34歳男性が2か月前からの右眼腫瘤で受診した。右眼の球結膜鼻側に径12mmの乳頭状有茎腫瘍があり，瞼結膜に4個のやや扁平な有茎状腫瘍があった。経過：切除した腫瘍は，術中の迅速組織診で扁平上皮癌と診断され，術後の病理検査で低分化ないし中分化した乳頭腫の診断が確定した。検体には，ヒトパピローマウイルスDNAハイブリダイゼーション法で，ローリスク群が陽性であった。結論：結膜乳頭腫は，ローリスク群ヒトパピローマウイルス感染による可能性がある。

Abstract.　Background：It is difficult to differentiate conjunctival papilloma from squamous cell carcinoma. Purpose：To report a case of conjunctival papilloma with human papillomavirus detected from the lesion. Case：A 34-year-old male presented with a tumor in his right eye since 2 months before. Findings：The right eye showed a papillomatous pedunculated tumor of 12 mm in diameter in the nasal sector of bulbar conjunctiva. Four similar lesions were present in the bulbar conjunctiva. The dissected tumor was tentatively diagnosed with squamous cell carcinoma by rapid intraoperative histopathological study. It was later diagnosed with mild to moderate differentiated papilloma. The specimen was positive for low-risk human papillomavirus by DNA hybridization. Conclusion：Conjunctival papilloma may be due to infection by low-risk human papillomavirus.
