要約 目的:涙囊炎がある乳幼児と成人から検出された原因菌と薬剤感受性の報告。対象と方法:2009年までの5年8か月間に検索した涙囊炎患者119例を対象とした。内訳は生後1~27か月の乳幼児56例66側と,44~94歳の成人63例73側で,平均年齢は乳幼児が7.1か月,成人が73.8歳である。結果:乳幼児から検出された110株では,Staphylococcus epidermidis,Streptococcus mitis,Streptococcus pneumoniae,Haemophilus influenzaeの順で多く,成人からの111株では,Staphylococcus epidermidisとmethicillin-resistant Staphylococcus aureus(MRSA)を含めた黄色ブドウ球菌が約半数を占めた。高頻度にみられたStaphylococcus epidermidisは,中間感受性を含んだレボフロキサシンへの耐性が,乳幼児では100%,成人では80%にあった。結論:涙囊炎の原因菌は,抗菌点眼薬の長期投与で耐性化している可能性がある。
Abstract. Purpose:To report the microorganisms and their drug-resistance in infants and adults with dacryocystitis. Cases and Method:This study was made on 119 patients of dacryocystitis seen during 5 years 8 months through 2009. The series comprised 66 affected sites of 56 infants and 73 sites of 63 adults. The age ranged from 1 to 27 months in infants and from 44 to 94 years in adults. Results:A total of 110 strains were isolated from infants. They comprised Staphylococcus epidermidis,Streptococcus mitis,Streptococcus pneumoniae and Haemophilus influenzae in the descending order. A total of 111 strains were isolated from adults. Staphylococcus epidermidis showed resistance to levofloxacin in 100% of infants and 80% of adults. Conclusion:Prolonged use of antibiotics for dacryocystitis may have induced drug-resistance in causative microorganisms.
