要約　目的：新診断基準による眼病変からのサルコイドーシスの診断と評価。対象と方法：過去8年間に当科ぶどう膜炎外来でサルコイドーシスが疑われた197例を新診断基準で検討した。結果：サルコイドーシスと確定診断された97例のうちの臨床診断群52例と，確定診断ができなかった疑い群100例の比較では，眼病変に相違はなかったが，全身検査での陽性率に差があった。新基準で新たに「疑い群」から「臨床診断群」になった26例では，全例に両側肺門リンパ節腫脹（BHL）があった。一方，旧基準で「臨床診断群」であった症例中9例が，BHL陰性のため「疑い群」になった。結論：新診断基準ではBHL陽性による診断がつけやすくなったため，診断率が向上した。BHL陰性例の診断は今後の問題である。

Abstract.　Purpose： To evaluate the diagnosis of sarcoidosis from ocular findings based on revised version of diagnositic guidelines for sarcoidosis（2006）. Cases and Method：This study was made on consecutive 197 cases suspected of sarcoidosis in our uveitis clinic during the past 8 years. Results：According to the new diagnostic criteria,97 cases were diagnosed with sarcoidosis,of whom 52 cases were based on clinical features. The remaining 100 cases were diagnosed as sarcoidosis suspect. These two groups showed similar findings regarding ocular manifestations and showed difference in systemic features. Twenty-six cases of sarcoidosis suspect by the earlier criteria were assigned to clinical sarcoidosis by the new guideline. All the 26 cases showed bilateral hilar lymphadenopathy（BHL）. Nine cases were assigned to sarcoidosis suspect because of negative BHL findings. Conclusion：Diagnosis of sarcoidosis became easier by inclusion of BHL hypertrophy in the new criteria.
