要約　目的：単純ヘルペスウイルスによる急性網膜壊死が5年前に僚眼にも発症していたと推定される若年例の報告。症例：19歳女性が左眼の充血と眼痛で受診した。5年前に網膜剝離があり，硝子体手術を受けていた。矯正視力は右50cm手動弁，左（1.2）であった。右眼にはシリコーンオイルが充填され，虹彩後癒着が全周にあった。左眼には虹彩炎，硝子体混濁，眼底下方に黄白色滲出斑があった。経過：前房水のPCR検査で単純ヘルペスウイルスが検出され，これによる急性網膜壊死と診断した。即日入院のうえ，アシクロビル点滴と副腎皮質ステロイドの全身投与を行った。左眼視力は（0.1）に低下したが，3週後に（0.9），1年後の現在（1.2）に回復し，再発はない。結論：急性網膜壊死が疑われる症例では，房水のPCR検査などによる早期の診断と治療が望まれる。

Abstract.　Purpose：To report a case of acute retinal necrosis due to herpes simplex virus involving fellow eye with a 5-year interval. Case and Findings：A 19-year-old female presented with hyperemia and pain in her left eye. She had had retinal detachment and received vitrectomy 5 years before. Her corrected visual acuity was hand motion right and （1.2） left. Her right eye showed silicone oil in the vitreous. Her left eye showed signs of iritis, vitreous opacity and exudates in the periphery. Clinical Course：Aqueous humor was aspirated on the day of her visit. Polymerase chain reaction（PCR）showed herpes simplex virus, leading to the definite diagnosis of acute retinal necrosis. Systemic acyclovir and corticosteroid resulted in temporary decrease of left visual acuity to （0.1） but in final improvement to （1.2） one year later. Conclusion：Anterior chamber tap processed by PCR may contribute to early diagnosis and treatment in suspected cases of acute retinal necrosis.
