We tested the current and newly proposed guidelines for the diagnosis of ocular sarcoidosis in 159 patients of uveitis seen during the foregoing 35 months. The series comprised 78 patients of histologically diagnosed sarcoidosis and 81 patients of other uveitis entities including 40 patients of Behçet disease and 21 patients of Vogt-Koyanagi-Harada disease. The current guidelines showed sensitivity of 84.6% and specificity of 38.3% for sarcoidosis. The newly proposed guidelines showed sensitivity of 83.3% and specificity of 60.5% for sarcoidosis by assuming 2 or more items for diagnostic criteria,and sensitivity of 69.2% and specificity of 87.7% by assuming 3 or more items for the criteria. The findings show that,if sensitivity is a major factor,a case may be diagnosed with sarcoidosis when 2 or more categories are fulfilled. If specificity is a major factor,a case may be diagnosed with sarcoidosis when 3 or more categories are fulfilled.
