Behçet病にインフリキシマブを投与した3例

Three cases of Behçet disease treated by systemic infliximab

要約　目的：インフリキシマブで治療したBehçet病3症例の報告。症例：症例はすべて男性で，それぞれ34歳，35歳，42歳である。Behçet病の眼発作が始まってから，7年，13か月，30か月の間，コルヒチンとシクロスポリンで加療したが，ぶどう膜炎の発作が続いていた。体重1kgあたり5mgのインフリキシマブの全身投与を開始した。最初は0，2，6週間の投与とし，以後は8週間ごとに投与した。結果：3例ともインフリキシマブ投与前の発作時視力が0.1以下であったのが，投与後は0.6ないし1.2に改善した。年あたりの発作回数は，17回が0回，2回が0回，3回が2回になった。結論：インフリキシマブはBehçet病の難治性ぶどう膜炎に有効であった。

Abstract.　Purpose：To report the outcome of systemic treatment with infliximab in 3 cases of Behçet disease with ocular involvement. Cases：Patients were all males aged 34,35 and 42 years respectively. Treatment with systemic colchicine and cyclosporine for 7 years,13 months and 30 months respectively failed to prevent acute attacks of uveitis. Treatment was then switched to systemic infliximab at the dosis of 5 mg/kg. Infliximab was given for 0,2,6 weeks to be followed by an interval of 8 weeks thereafter. Results：Visual acuity during acute attacks was below 0.1 in all cases before switching to infliximab. It improved to 0.6,1.0 and 1.2 after switching. Frequency of acute ocular attacks per year improved from 17 times to nil,twice to nil,and thrice to twice respectively. Conclusion：Systemic treatment with infliximab was effective in preventing or alleviating acute attacks of uveitis in 3 cases of Behçet disease.
