正常眼圧緑内障に対するニプラジロール点眼単独投与の臨床効果

Effect of monotherapy with nipradilol ophthalmic solution for normal-tension glaucoma

要約　目的：正常眼圧緑内障46例に対するニプラジロール点眼単独投与の効果報告。症例と方法：10か月間に3施設で正常眼圧緑内障と診断した46例46眼を対象とした。男性18例，女性28例で，年齢は35～83歳（平均65歳）である。ニプラジロール点眼のみで治療し，6～48か月（平均31か月）の経過を観察した。眼圧は6か月ごとに測定し，視野障害はハンフリー視野計の30-2プログラムを用い，3dB以上の悪化を進行とし，Kaplan-Meier生命表で判定した。治療開始前の眼圧は平均15.7±3.2mmHgであった。結果：点眼開始6～36か月の間，眼圧は有意に低下した。視野障害の4年生存率は83.2％であった。結論：正常眼圧緑内障に対するニプラジロール単独点眼には，眼圧下降と視野障害の進行抑制効果がある。

Abstract.　Purpose：To report the effect of nipradilol ophthalmic solution for normal-tension glaucoma. Cases and Method：This prospective study was performed on 46 eyes of 46 patients who were diagnosed with normal-tension glaucoma in three institutions. The series comprised 18 males and 28 females. Their age ranged from 35 to 83 years,average 65 years. They were treated by topical nipradilol alone. Intraocular pressure（IOP）was measured every 6 months. Visual field was evaluated by program 30-2 of Humphrey perimeter. Deterioration by more than 3dB was regarded as progression. Baseline IOP averaged 15.7±3.2mmHg. Results：IOP decreased significantly until 36 months after start of treatment. When judged by life table of Kaplan-Meier,the visual field showed 4-year survival of 83.2％. Conclusion：Monotherapy with topical nipradilol is effective for IOP reduction and prevention of progression of visual field defect in normal-tension glaucoma.
