要約　目的：抗カルジオリピン抗体が陽性であった乳頭血管炎の症例の報告。症例と所見：31歳女性が1週前からの左眼霧視で受診した。矯正視力は左右とも1.5であり，左眼に視神経乳頭血管炎と切迫型網膜中心静脈閉塞症があった。プレドニゾロン内服3週後に乳頭浮腫と網膜出血は消退した。血液検査で抗カルジオリピンIgG抗体が陽性で，ループスアンチコアグラントと抗β₂-glycoproteinⅠ抗体は陰性であった。結論：本症例の検査所見は抗リン脂質抗体症候群の診断基準は満たさなかったが，眼病変の発症にはこれと関係した何らかの自己免疫機序が関係している可能性がある。

Abstract.　Purpose：To report a case of optic disc vasculitis with positive anticardiolipin antibody. Case：A 31-year-old woman presented with blurring in her left eye since one week before. Her corrected visual acuity was 1.5 in either eye. Her left eye showed impending central retinal vein occlusion with vasculitis of the optic disc. The findings normalized after 3 weeks of peroral prednisolone. Laboratory studies showed positive anticardiolipin antibody, negative anti-β₂ glycoproteinⅠantibody, and negative lupus anticoagulant. Conclusion：This case failed to fulfill the diagnostic criteria for antiphospholipid syndrome. Still, there is a possibility for the ocular lesion to be related with a similar disorder of the anto-immune system.
