Abstract. Purpose:To report a case of iridocyclitis caused by varicella-zoster virus(VZV). Case:A 42-year-old woman presented with hyperemia and pain in her left eye since 9 days before. Findings:Visual acuity was 1.2 in either eye. Intraocular pressure(IOP)was 17 mmHg right and 24 mmHg left. The left eye showed findings of acute anterior uveitis with mutton-fat keratic deposits. Fluorescein angiography showed dye leakage from retinal vessels in the periphery. Polymerase chain reaction(PCR)of aqueous sample showed VZV. Simple herpes virus and cytomegalovirus were negative. Topical treatment was followed by normalized IOP and iris 3 weeks later. Ciliary injection developed another 10 days later which was controlled by topical acyclovir leaving hyperpigmentation in the chamber angle. Conclusion:Acute anterior uveitis may be caused by VZV without manifestations in the skin. PCR of aqueous humor facilitates early diagnosis.
