　27歳女性が視力低下と羞明で受診した。14歳の頃から両眼の羞明と色の判別困難があった。家族内に同様の疾患はなく，両親の血族関係もない。前医では，通常の白色閃光全視野刺激による暗順応網膜電図（ERG）で異常がなく，中心暗点があることから，occult macular dystrophyが疑われていた。矯正視力は左右とも0.1であり，眼底ならびに蛍光眼底造影で異常所見はなかった。改めて全視野刺激による暗順応網膜電図を検査した。杆体系反応と杆体・錐体混合反応は正常であったが，錐体系反応は消失していた。Occult macular dystrophyでは錐体系反応が正常で，黄斑部局所または多局所網膜電図のみが異常を示すことから，本症例がcone dystrophyであると診断した。

　A 27-year-old woman presented with impairment of vision and photophobia. Photophobia and difficulty in color discrimination had started at the age of 14 years. She had negative family history. Occult macular dystrophy had been suspected by the referring ophthalmologist because of normal fundus finding,central scotoma,and normal electroretinogram（ERG）elicited by strong full-field flash. Her corrected visual acuity was 0.1 in either eye. Both eyes showed normal funduscopic and fluorescein angiographic findings. Full-field ERG showed normal rod and rod-cone responses and absence of photopic or cone response. Occult macular dystrophy is characterized by normal cone ERG and by abnormal macular or multifocal ERG. We therefore diagnosed this patient with cone dystrophy.
