　64歳男性が2日前に右眼視力低下を自覚して受診した。矯正視力は右0.3，左1.5であった。右眼の視神経乳頭が発赤・腫脹し，軟性白斑と出血がその周囲にあった。光干渉断層計（OCT）で，中心窩に網膜剝離，乳頭黄斑間に網膜外層の膨化があった。フルオレセインとインドシアニングリーン蛍光造影で乳頭に色素漏出があったが，網膜血管と黄斑部には異常所見がなかった。3週後の中心窩剝離と網膜外層膨化の吸収に伴い，星芒状白斑が出現し，特発性星芒状視神経網膜炎（Leber）と診断した。視力は1.0に改善した。視神経乳頭からの強い漏出が黄斑浮腫と中心窩剝離の原因であると推定した。星芒状白斑は黄斑浮腫の吸収過程で出現した。

　A 64-year-old male presented with failing vision in his right eye since two days before. His corrected visual acuity was 0.3 right and 1.5 left. The right eye showed hyperemia and swelling of optic disc surrounded by soft exudates and hemorrhages. Optical coherence tomography（OCT）showed retinal detachment in the fovea and swelling of outer retinal layers along the papillomacular bundle. Fluorescein and indocyanine green angiography showed apparently intact retinal vessels and dye leakage in the optic disc. Star figures appeared in the macula 3 weeks later when foveal detachment and retinal edema started to resolve. This finding led to the diagnosis of Leber's idiopathic stellate neuroretinitis. The final visual acuity improved to 1.0. Exudation from the optic disc appeared to be the cause of macular edema with foveal detachment.
