組織プラスミノーゲン活性化酵素を用いてケミカルビトレクトミーを行った若年性外傷性網膜剝離の2例

Two juvenile cases of traumatic retinal detachment treated by chemical vitrectomy utilizing tissue plasminogen activator

　後部硝子体が剝離していない外傷性裂孔原性網膜剝離2眼を加療した。いずれも男性で，年齢は15歳と18歳であった。硝子体内に組織プラスミノーゲン活性化酵素を16μg，8,000単位注入して後部硝子体剝離を作製した。その1時間後に行った硝子体手術では，硝子体を十分かつ安全に郭清でき，両症例とも網膜が復位した。組織プラスミノーゲン活性化酵素で後部硝子体剝離を誘発することができ，若年者の網膜剝離に対する硝子体手術の安全性を向上することが期待できる。術後6か月間の経過は良好であるが，網膜への毒性にはなお注意が必要である。

　We treated traumatic rhegmatogenous retinal detachment in two males aged 15 and 18 years. The posterior vitreous was still attached to the retina. Artificial posterior vitreous detachment（PVD）was induced by intravitreal injection of tissue plasminogen activator（16μg,8000IU）. Vitreous surgery started one hour later. The vitreous could be sufficiently and safely excised,resulting in retinal reattachment. This case illustrates that tissue plasminogen activator,or chemical vitrectomy,may be a useful adjuvant to vireous surgery for traumatic retinal detachment in juveniles. Long-term observation is needed to rule out the possibility of retinal toxicity.
