網膜中心動脈分枝閉塞症を併発したLeber特発性星芒状視神経網膜炎の1例

Branch retinal artery occlusion in a case of Leber's idiopathic stellate neuroretinitis

　原因不明と思われた片眼性乳頭浮腫を初発症状とし，経過中に黄斑部星芒状白斑を伴い自然寛解傾向を示したLeber特発性星芒状視神経網膜炎と考えられる61歳男子例を経験した。多くは，自然寛解するとされているが，限局性網脈絡膜炎や前部虚血性視神経症が合併し，炎症のみならず，網脈絡膜循環障害が伴うものと考えられる。本症例では，経過中，網膜中心動脈分枝閉塞症を併発し，幸いにも視力は1.0を保つことができたが，下水平半盲の視野欠損を起こした。本症例の経過を通じて，Leber特発性星芒状視神経網膜炎のような疾患単位を念頭におきつつ，さらなる注意深い経過観察が必要であると思われた。

A 61-year-old man developed optic disc edema in the right eye. No underlying cause could be identified. Stellate macular exudates appeared 6 weeks later. The disc edema resolved spontaneously with improvement in visual acuity. Branch retinal artery occlusion developed in the same eye 2 months later, resulting in permanent visual field defect in the inferior hemisphere. While the initial clinical features were compatible with those of Leber's idiopathic stellate neuroretinitis, associated retinochoroidal circulatory disturbance is a possibility in this disease entity.
