目的：特発的に眼窩内壁が篩骨洞側に膨隆し，眼球陥凹が起こった症例の報告。症例：76歳男性が6か月前からの左眼の眼球陥凹で受診した。頭部への外傷や，鼻または副鼻腔手術の既往はなかった。所見：左眼には右眼よりも2mmの差がある眼球陥凹と上眼瞼下垂があった。眼球運動は正常で複視はなかった。CT検査で左眼窩の内壁と眼窩内容が篩骨洞側に膨隆していた。本人が希望せず，手術は行わなかった。結論：本症例でみられた特発的な眼窩内壁の篩骨洞側への膨隆は，加齢に伴う眼窩内壁の脆弱化が原因であると推定した。眼球陥凹にはこのような状態が鑑別診断の1つとして考慮されるべきである。

Purpose：To report a case of spontaneous enophthalmos and bulging of medial orbital wall towards the ethmoid sinus. Case：A 76-year-old male presented with enophthalmos of his left eye since 6months before. He had had no history of trauma or surgery to the head,nose,or paranasal sinus. Findings：The left eye showed enophthalmos by 2mm when compared with the right eye. Ptosis of upper eyelid was also present. He showed normal ocular motility and had no diplopia. Computed tomography showed bulging of medial orbital wall and soft orbital tissue towards the left ethmoid sinus. Surgery was refused by the patient. Conclusion：Age-related weakening of bony orbit may have induced bulging of medial orbital wall in the present case. This condition is to be included in the differential diagnosis of enophthalmos.
