Abstract Purpose:To report a case of enophthalmos caused by orbital metastasis of breast carcinoma.

Case:A 53-year-old woman who had undergone a left mastectomy, chemotherapy and radiotherapy 6 years ago for breast lobular carcinoma, presented with right ptosis that gradually developed in at least 1-year duration. Her visual acuity was 1.0 bilaterally. Enophthalmos, ptosis, induration of the periorbital skin, and restricted eye movements were observed in her right eye. Orbital CT scan showed a diffuse, infiltrative mass encircling the right globe. Orbital skin biopsy revealed the same pathology as her previously treated breast carcinoma. Letrozole and radiotherapy improved her symptoms. Four years later, her visual acuity decreased to 0.5 because of submacular fluid caused by optic disc swelling from enlarged orbital metastasis. CDK4/CDK6 inhibitor and aromatase inhibitor were prescribed to her, and her visual acuity improved.

Conclusion:Enophthalmos is a relatively rare symptom of orbital metastasis of carcinoma;however, breast carcinoma is the most probable primary pathology. Appropriate treatments can improve the symptom and patient's quality of life.
