Abstract. A 57-year-old female presented with progressive blurring and pain in her left eye since 8months before. Her right eye had been enucleated after trauma. Her visual acuity was 0.8 left which deteriorated to 0.03 three days later. She was incapable of lid opening. The ocular motility was restricted in all directions. Diagnostic imagings showed malignant tumor in the sphenoidal sinus extending to the left orbit. She was treated by radical removal of tumor in the sphenoidal and ethmoidal sinus followed by systemic prednisolone. Blepharoptosis and ocular motility rapidly improved. The tumor was identified as diffuse malignant macrocellular B lymphoma. Her visual acuity improved to 1.0 but was reduced to 0.03 due to optic atrophy. This case illustrates that orbital apex syndrome may be the initial manifestation of malignant lymphoma originating in the sphenoidal sinus.
