チモロールゲルからレボブノロールへの切り替えによる効果

Effect of levobunolol switched from timolol gel-forming solution

目的：0.5％イオン応答ゲル化剤添加チモロール点眼薬（チモロール）をレボブノロール1日1回点眼に変更した際の眼圧への効果と使用感の検討。方法：チモロールを3か月以上使用していた緑内障35例35眼を対象とした。内訳は男性13例，女性22例であり，原発開放隅角緑内障24眼，正常眼圧緑内障7眼，原発閉塞隅角緑内障3眼，高眼圧症1眼である。眼圧は1か月ごとに3か月間測定した。結果：変更前の眼圧は17.1±1.9mmHgであり，変更2か月後で眼圧は有意に下降した。1か月後と3か月後の眼圧は変更前の眼圧と有意差がなかった。チモロール点眼で「べとつく」または「ぼやける」と答える例が多かった。18例（51％）がチモロールよりレボブノロールを好み，2例（6％）がチモロールを好み，15例（43％）が「どちらでもよい」と答えた。結論：緑内障の点眼治療薬としてチモロールからレボブノロールに切り替えた結果，3か月まで眼圧が変化せず，レボブノロールによる使用感が概してよかった。

Purpose：To evaluate ocular hypotensive effect and comfort before and after switching 0.5％ timolol gel-forming solution to levobunolol once daily. Method：This prospective study involved 35 eyes of 35 glaucoma patients. The series comprised 13 males and 22 females and included primary open-angle glaucoma 24 eyes,normal-tension glaucoma 7 eyes,primary angle closed glaucoma 3 eyes and ocular hypertension one eye. Intraocular pressure（IOP）was measured once a month for 3 months. Results：IOP before switching averaged 17.1±1.9 mmHg. It decreased significantly 2 months after switching. There was no difference from baseline value 1 and 3 months after switching. Patients complained of stickiness and blurred vision before switching. Levobunolol was preferred to timolol by 18 patients（51％）. Conclusion：There was not difference in IOP before and after switching timolol to levobunolol up to 3 months. Ocular comfort was better while using levobunolol than timolol gel-forming solution.
