正常眼圧緑内障患者におけるニプラジロール点眼3年間投与の効果

Effect of nipradilol instillation for three years in eyes with normal-tension glaucoma

要約　目的：正常眼圧緑内障に対しニプラジロールを3年間点眼した結果の報告。対象と方法：新規に0.25％ニプラジロール点眼薬を単剤として処方し，3年間の経過を追えた正常眼圧緑内障31例31眼を対象とした。男性14例，女性17例であり，年齢は29～76歳（平均52歳）であった。1か月毎に眼圧を測定し，1年毎に視野と視神経乳頭を記録し解析した。結果：眼圧は点眼開始前が16.9±1.7mmHg，点眼3年後が14.0±1.9mmHgであり，有意に下降した（p＜0.0001）。ハンフリー視野計でのmean deviation値とpattern standard deviation値，ハイデルベルグ網膜断層計（HRTⅡ）のすべてのパラメータ値には，点眼前と点眼1，2，3年後で有意差がなかった。結論：正常眼圧緑内障眼にニプラジロール点眼薬を3年間投与すると，眼圧は有意に下降し，視野と乳頭形状は変化しなかった。

Abstract.　Purpose：To report the outcome of topical nipradilol for 3 years in eyes with normal-tension glaucoma. Cases and Method：This study was made on 31 eyes of 31 patients with normal-tension glaucoma who received 0.25％ nipradilol for 3 years. The series comprised 14 males and 17 females. Their age ranged from 29 to 76 years, average 52 years. Intraocular pressure（IOP）was measured every month. Visual field and shape of optic disc was monitored once a year. Results：Baseline IOP averaged 16.9±1.7mmHg. It averaged 14.0±1.9mmHg at 3 years. The difference was significant（p＜0.0001）. There were no significant differences in mean and pattern standard deviations by Humphrey perimetry and in all the parameters by Heidelberg Retina Tomograph Ⅱ over the 3-year period. Conclusion：Instillation of nipradilol induced significant IOP decrease with no changes in visual field or morphology of optic disc.
