目的：球結膜に原発したと推定される眼窩扁平上皮癌1例の報告。症例：79歳女性。3歳のとき外傷で右眼が失明し，25歳のときから義眼を装用していた。最近，上眼瞼に腫瘤が生じ，義眼が入らなくなった。生検で高分化型扁平上皮癌と診断された。画像検査で右眼窩に腫瘤があり，腫瘤と眼球を摘出した。腫瘤の断端が病理診断で陽性であり，術後に総量46 Gyの放射線照射を行った。結果：手術から9か月後の現在まで再発がなく，経過は良好である。結論：眼窩扁平上皮癌の治療では，まず腫瘍を全摘し，断端が陽性であれば放射線照射を併用することが有用である。

Purpose：To report a case of squamous carcinoma of the orbit originating from the bulbar conjunctiva. Case：A 79-year-old woman had lost vision in her right eye due to trauma at the age of 3 years. She had been wearing artificial eye since the age of 25 years. She visited us complaining of a tumor mass in the upper eyelid followed by difficulty in inserting the artificial eye. Biopsy led to the diagnosis of well-differentiated squamous cell carcinoma. Diagnostic imaging showed a tumor mass in the right orbit. She was treated by surgical removal of tumor and eyeball. Remnant portion of the tumor received radiation totalling 46 Gy. Result：The patient has been doing well without recurrence during 9 months after surgery. Conclusion：Total excision is the treatment of choice for squamous carcinoma of the orbit,to be followed by radiation when indicated.
