目的：裂孔原性網膜剝離に対する手術成績の報告。対象と方法：3年間に裂孔原性網膜剝離108例110眼に初回手術を行った。51眼には強膜内陥術，59眼には硝子体手術を行った。結果：強膜内陥術を行った51眼中，45眼（88％）が初回手術で復位し，最終的に全例が復位した。硝子体手術を行った59眼中，52眼（88％）が初回手術で復位し，最終的に58眼（98％）が復位した。両群を合わせた初回復位例は110眼中97眼（88％）であり，最終的に109眼（99％）が復位した。最終視力は，硝子体手術群で視力改善例が多かった。結論：裂孔原性網膜剝離に対する手術法の選択では，術前検査の結果と患者の年齢などを考慮すべきである。

Purpose：To report the outcome of surgery for rhegmatogenous retinal detachment in our institution. Cases and Method：Primary surgery for rhegmatogenous retinal detachment was performed on 110 eyes of 108 patients during a 3-year period. Scleral buckling was performed on 51 eyes. Vitreous surgery was performed on 59 eyes. Results：Retinal re-attachment was obtained in 45 eyes（88％）after initial scleral buckling and in 52 eyes（88％）after initial vitreous surgery. Ultimate retinal re-attachment was obtained in 109 eyes（99％）. The postoperative visual acuity was better in eyes receiving vitreous surgery. Conclusion：Preoperative findings and the age of patients are major factors in the selection of surgical procedure for rhegmatogenous retinal detachment.
