ぶどう膜炎に伴う嚢胞様黄斑浮腫に対する硝子体手術の有効性

Effect of vitreous surgery for cystoid macular edema secondary to uveitis

（19-レセ1-8）　嚢胞様黄斑浮腫（CME）を伴うぶどう膜炎の4症例に対して，インフォームド・コンセントを得た後，硝子体手術を行った。4例ともステロイドあるいはシクロスポリンの全身投与に抵抗性を示すか，全身的副作用の問題で継続投与不可能な症例であった。3例ではCMEの著明な改善をみたが，1例では不変であった。視力は全例で改善した。以上の結果から，薬物療法に抵抗するぶどう膜炎のCMEに対して，硝子体手術は一治療手段となりうるものと思われた。

We performed vitreous surgery for 4 eyes of uveitis with cystoid macular edema after informed consent. Systemic corticosteroid or cyclosporin proved futile because the eyes failed to respond or because the medications had to be discontinued due to severe systemic complications. Following surgery, cystoid macular edema improved in 3 eyes and persisted in one eye. Visual acuity improved in all the eyes. The findings show that vitreous surgery may be regarded as one of the alternative therapy for cystoid macular edema in uveitis which cannot be controlled by systemic medications.
