Purpose:To describe a case of antiphospholipid antibody syndrome who developed occlusions of retinal artery and vein. Case:A 59-year-old female had an acute occlusion of branch retinal artery and central retinal vein in her right eye. When seen 2 days later,her visual acuity was 0.01 right and 1.0 left. Radial optic neurotomy 4 days later was followed by central retinal artery occlusion. She showed thrombocytopenia and anticardiolipin antibody and was diagnosed with antiphospholipid antibody syndrome secondary to systemic lupus erythematosus. Systemic corticosteroid induced improvement of thrombocytopenia. Thrombolytic treatments was followed by improved circulation in retinal artery but not in the vein,resulting in final visual acuity of 0.03. Conclusion:Antiphospholipid antibody syndrome may be one of the causes of central retinal vein occlusion in the elderly.
