6年前に網膜色素変性症と診断された49歳女性が，2か月前からの右眼視力低下で受診した。矯正視力は右0.1，左0.6であった。右眼に黄斑上膜を伴う全層黄斑円孔があった。硝子体手術を行い，インドシアニングリーンを併用した内境界膜切除を行った。術後8日目に円孔は閉鎖したが，以前からあった輪状暗点が内方に拡大してきた。術後14か月目にも中心暗点があり，右視力は0.03であった。視力と視野が術後に悪化したことに，内境界膜を剝離するときの機械的侵襲，眼内照明による光傷害，インドシアニングリーンによる網膜色素上皮障害の可能性がある。

A 49-year-old woman presented with decreased visual acuity in her right eye since 2 months before. She had been diagnosed with retinitis pigmentosa 6 years before. Her corrected visual acuity was 0.1 right and 0.6 left. Her right eye showed full-thickness macular hole with epiretinal membrane. She received vitreous surgery with peeling of internal limiting membrane with adjunct use of indocyanine green. Following closure of macular hole 8 days after surgery,the ring scotoma started to enlarge. A large area of absolute central scotoma had formed 14 months after surgery,with visual acuity of 0.03. The observed deterioration in visual field may be secondary to mechanical insult to the retina,phototoxicity by endoillumination,or injury to the retinal pigment epithelium by indocyanine green.
